Description: There is significant controversy over the use of early endoscopy in caustic injuries because it is an invasive procedure and it remains unclear what diagnostic value it has in the early treatment of such injuries. There is clearly a role for endoscopy in the assessment and treatment of established severe caustic injuries that require operative management. However, it remains unclear if there is value in early endoscopy, in addition to patient history and their clinical condition, to determine if they will develop significant complications, e.g. oesophageal strictures. Clinical experience and case series demonstrate that patients who develop early complications (perforation) or later complications such as strictures, ingest strong acids and alkalis and have significant clinical findings on admission. In addition, a number of paediatric studies support no intervention in children who remain asymptomatic during the initial assessment period.
